format=json output non-json

Using --format=json should output only json data (or in case that's not possible bc of parallel processing, multiple jsons with 1 per line is acceptable too)

But outputting "FILE: whatever.php" kind of counter the purpose of using json format
The file path should be included in the json itself

EDIT: and also no blank lines
